QUALIFICATIONS1.Bachelors degree or certification in physical therapy, with Master or Clinical Doctorate in physical therapy preferred.2.Current state licensure in states where employed.3.one year clinical background as a Physical Therapist4.Current CPR Card5.TB Skin Test Results within past year OR Chest X Ray6.2 Forms of I.D.Preferably Drivers License and SSN Card7.Immunization RecordsIF APPLICABLEFUNCTIONS1.PRN or full-time hours depending on therapist availability2.Demonstrate and articulate sound clinical reasoning in synthesizing complex evaluation data identifying diagnosis and developing a comprehensive plan of care.3.Develop and update appropriate plan of care in collaboration with the patientfamily and related to the persons age and lifestyle with measurable long and short-term goals and a defined DC plan.4.Understand the role of physical therapy in the context of the patients age, total needs perspective and environment.5.Perform age appropriate competency skills in area of practice and with respect to individual patients problems.6.Follow intervention protocols and alter plan of care appropriately.7.Discharge patient when goals have been met or medical necessity is no longer warrants intervention.8.Establish provisions for continuing servicesfollow up as needed.9.Utilize support personnel Aides, Sr. Aides, and Assistants as appropriate and available.10.Ensure that patientfamily understands when and how to assess further intervention.11.Documentation is concise, pertinent, and meets the professions documentation standards. 12.Adapting standard techniques depending on medical conditions associated with disability, illness or trauma.13.Supervising assistant practitioners appropriate training.14.Understanding and observing health and safety at work and welfare issues.15.Reporting incidents of injury to self or others according to department procedures.16.Applies knowledge and principles of Universal Precautions, aseptic and sterile techniques, to minimize exposure tospread of infectious organisms.
