Gentiva also allows for flexibility in terms of work schedulesmake your own schedule add balance to your life As a bTherapy Team Leaderb, you will brDirect careservices provided and developmentimplementation of patient plans of care and physician protocols for Specialty patients within scope of practice, to include performing admissions screening, clinical assessments and treatments as appropriate, consulting with 3rd party representatives regarding patient financial responsibility, providing case management and follow up, to ensure care plan stated outcomes are achieved. brProactively facilitate coordination of ancillaryparaprofessional services and assists with coordination of disciplines and utilization of community resources, including transition of care from a facility to place of residence as needed. Oversee and assist in evaluating the performance of all teammates which may include therapists, social workers, nurses and HHAs. Collaborate with teammates to ensure an appropriate match between patient needs and the skillsabilities of the care team. brCoordinate carecommunication between teammates, attending physicians, appropriate administrative staff, referral sources and case managers to ensure appropriateness of care, and facilitates any necessary changes to the plan of care. Review and interpret trend over time datapatient progress reports and provides concise findings to physicians, ARNPs and PAs. brPromote specialty growth in revenue and profitability. Manage resource allocation careservices and supplies within budgetary and Care Protocol restraints or documents appropriate variances. Maintain marketplace Specialty info to assist the SpecialtyRehab Director in assessing business opportunities. Provide input to quarterlyannual businessmarket plans and budgets, cost analyses, monitors expenses and explains variances to budget. brAssist Sales team with Clinical review of Specialty for customers or any other related marketing duties, including meeting with potential customers to develop relationships. brCollaborate with RN Clinical Manager to manage the relationships between service demands and availability of Specialty credentialed staff. brParticipate in the recruitmentinterviewingselectionorientation of teammates, evaluates their performance relative to job goalsrequirements, coaches staff recommending in-service education programs and ensures adherence to internal policiesstandards. Promote ongoing Specialty development through on-site observation of care delivery. Provide Specialty trainingcredentialing of all Specialty teammates.bQualificationsb brDegree and licensure in the state s of practice for either physical therapist, occupational therapist or speech language pathologist with a min of 3yrs clinical background required to include home health, hospital or skilled nursing facility background in at least 1 of the following Rehabilitation, Medical Surgical or Sub-specialty Units such as Oncology, Endocrine, Orthopedic, Cardiopulmonary, Gerontology or Neurology preferred. brExperience leading an interdisciplinary carerehabilitation team and demonstrated awareness of individual disciplines roles and abilities required. brExperience in a communityhomecare setting and in the Specialty of the Team Leader role preferred.
